Catalyst is a web based electronic clinical case and management software, that offers a fully integrated and comprehensive electronic health records and information management system, to all community addiction treatment service providers (HSPs) funded by the Province of Ontario.
The system is used by these HSPs to assess clients, plan treatment and manage client addiction treatment services. Catalyst also provides a platform for information sharing among HSPs to assist in referrals and coordination of addiction treatment services to consumers.
What is it – An evidence-based process for screening and assessing individuals accessing care in the addiction sector that assists the accurate identification of their needs, assists in developing a treatment plan, and in matching them to the most appropriate level and type of care.
What is it – The OPOC-MHA tool is a standardized way for gathering client feedback on the quality of care received across both community and hospital settings. This brings the client’s voice forward as a source of evidence to support program, agency and system quality improvement efforts.
Community Business Intelligence (CBI) is an initiative that provides community health service providers (HSPs) an aggregate reporting tool for the purposes of reporting at the individual, HSP, sector and LHIN level.
